Assumption of Risk: Fishing, boating, travel, docks, weigh-ins, and outdoor sporting events involve inherent risks, including injury, illness, property damage, disability, or death. I knowingly and voluntarily assume all risks associated with participating in Mater Academy's Grand Slam Challenge and related activities.
Release and Hold Harmless: To the fullest extent permitted by law, I release and hold harmless Mater Academy, its governing and affiliated entities, Reel Outdoorsman, tournament organizers, sponsors, hosts, property owners, officers, employees, volunteers, agents, and representatives from claims, liabilities, losses, damages, costs, or expenses arising from my participation, except where prohibited by law.
Safety and Legal Compliance: The captain is responsible for vessel seaworthiness, required insurance, and compliance with all U.S. Coast Guard requirements and federal, state, and local boating and fishing laws. Every participant must follow tournament rules, official instructions, weather advisories, and lawful safety directions. The captain has final responsibility for deciding whether to depart, continue fishing, or return to port.
Medical Authorization: In an emergency, I authorize event personnel to contact emergency services and obtain reasonably necessary medical care for me or a minor for whom I am the legal guardian. I understand that I remain responsible for resulting medical costs.
Equipment and Personal Property: I am responsible for my own vessel, vehicle, fishing gear, electronics, valuables, and other personal property. The released parties are not responsible for loss, theft, or damage except where liability cannot legally be waived.
Rules, Eligibility, and Results: I agree to follow the official tournament rules and accept decisions made by tournament officials regarding eligibility, fish classification, weigh-in, scoring, protests, safety, sportsmanship, and awards. Submission of this form does not confirm entry until payment and organizer acceptance are complete.
